    Objective: To explore the diagnostic and therapeutic approach of integrated traditional Chinese and western medicine (TCM-WM) on infectious multiple organs dysfunction syndrome/multiple system and organ failure (MODS/MSOF) for elevating the successful rate of rescuing the patients. Methods: Diagnosis with western medicine and Syndrome Differentiation of TCM in 225 in-patients of acute infectious disease complicated with MODS/MSOF were conducted, and TCM treatment, based on western medical comprehensive treatment, was given to observe the effect and explore the mechanism of the TCM-WM therapy. Results: Up to the end of 1998, 161 cases of the 225 cases were successfully cured and 64 died, the mortality being 28.4%. Among them, 58 out of 140 cases of MSOF died, the mortality was accounted for 41.4%. In 106 cases conformed to the diagnostic criteria of MSOF proposed by Professor Knaus WA, USA, 52 cases were cured successfully and 54 died, the mortality being 50.94%. Conclusion:TCM-WM treatment could elevate the therapeutic effect in treating MODS, the mechanism might be through improving the hemodynamic and hemorrheologic condition of patients to relieve nail-fold microcirculation disorder; influencing the levels of cytokine and inflammatory mediator, so as to alleviate the systemic inflammatory reaction, it might also abate the inhibited condition of gastro-intestinal motility, alleviate the intestinal flora imbalance, prevent intestinal bacteria and endotoxin malposition, and protect cells from peroxidation.

    Objective: To evaluate the therapeutic effect of Yangxin Fumai Oral Liquid (YFOL,养心复脉口服液), a Chinese herbal medicine for nourishing Heart and restoring pulse, in treating patients with extrasystole. Methods: The effect of YFOL was observed in treating 30 patients with different kinds of extrasystole and compared with that in 30 patients treated by propafenone. The effect of YFOL on experimental arrhythmia was studied in animals as well. Results: Clinical observation showed that the effect of treatment against extrasystole in the two groups was similar, but the YFOL group showed better results in symptom improvement (P<0.01) with no marked side-effect. Experimental study showed that YFOL could reduce the chloroform induced ventricular fibrillation occurrence in mice, delay the initiating time of ventricular extrasystole, tachycardia and fibrillation induced by aconitine, BaCl2 and coronary artery ligation in rats, or shorten the lasting time of arrhythmia, reduce the attacking frequency of ventricular extrasystole. There was significant difference in comparing with the control group (P<0.05, P<0.01). Conclusion:YFOL is a good and convenient Chinese herbal preparation for different kinds of extrasystole with few toxic and side-effect in clinical practice.

    Objective: To observe the clinical effect of Modified Taohong Siwu Decoction (MTSD) in treating pediatric intractable nephropathy (PIN). Methods: Ninety-five patients with PIN were divided into 2 groups at random. The 60 patients in the treated group were treated with MTSD and the 35 patients in the control group were treated with heparin. The clinical therapeutic effect and levels of thromboxane B2 (TXB2) and 6-keto-prostacyclin F1α (6-keto-PGF1α) before and after treatment were observed. Results: The total effective rate in the treated group was 81.7%, which was similar to that in the control group (80.0%, P>0.05). The levels of TXB2 and TXB2/6-keto-PGF1α ratio were higher in both groups of the patients as compared with those of the healthy control (P<0.01). After treatment, the two criteria were significantly improved in the two treated groups, as compared with those before treatment, the difference was significant (P<0.01), while in comparison between the MTSD treated group and the control group, no significant difference was found (P>0.05). Conclusion: MTSD has good effect in treating PIN, it could improve the metabolic unbalance of thromboxane and prostacyclin.

    To explore the therapeutic effect and mechanism of Bushen Huoxue Xiezhuo Decoction (BHXD) in treating minute lesion nephropathy (MLN) in rats. Methods: The MLN model rats established by a single intravenous injection of Adriamycin were divided into the model and the BHXD groups, and a normal group was set up for control. The effects of treatment on renal function, hemorrheologic parameters, renal tissue transforming growth factor β1 (TGF-β1) expression and poly-anion sites on glomerular basement membrane were observed dynamically. Results: After treatment, all the parameters between the BHXD group and the model group were significantly different respectively, morphological observation also showed the pathological changes in the BHXD group were milder than those in the model group. Conclusion: BHXD treatment could markedly improve the renal function, alleviate blood hypercoagulability and hyperviscosity, protect the anion barrier and delay the progression of glomerular fibrosis and sclerosis.

    To study the therapeutic effect of Chinese herbal medicines (CHM) in treating ascites and to elucidate its mechanism in regulating the lymphatic stomata and promoting the absorption of ascites from the peritoneal cavity. Methods: Using scanning electron microscope (SEM), computerized image processing and quantitative analytic assay, the effect of CHM extract, consisting of Atractylodes macrocephala, Salvia miltiorrhiza, Codonopsis pilosula, Alismatis orientale and Leonurus heterophyllus, was studied. Results: Intraperitoneal injection of nitric oxide (NO) supplier (Sodium nitroprussiate) or CHM administration could cause the average area of lymphatic stomata obviously enlarged (P<0.05), and the numbers of opening stomata significantly increased (P<0.01) in normal healthy mice. When L-notroarginine, a NO synthetase suppressor, was injected after CHM administration, the regulating effect of CHM on lymphatic stomata was inverted obviously, i.e. the average area and the density of lymphatic stomata were markedly reduced (P<0.01). Conclusion: CHM might treat ascites through increasing the endogenous NO concentration to open the lymphatic stomata and in turn to conduct the peritoneal water through lymphatic path.

    Urticaria is a common and frequently encountered disease, which is mainly manifested by extreme pruritus and lumpish eruption of the skin. Acute urticaria with a short disease course can be cured, while the chronic one with repeated attacks is a lingering disorder. In the past few years, 40 cases of urticaria were treated by cupping at the back-shu points of the five zang-organs and Geshu (BL 17). Another 20 cases in the control group were treated with Fang Feng Tong Sheng Wan (防风通圣丸Miraculous Pills of Ledebouriella) and chlorpheniramine maleate. The therapeutic effect in the treatment group was significantly superior to that in the control group. The results are reported as follows.

    Acupuncture treatment integrated with cupping for enhancing the therapeutic effect is a technique in Yang's school of acupuncture carried forward from generation to generation. The author's grandfather created Yang's shallow puncturing and cupping therapy to enhance the effect of promoting blood flow and removing blood stasis in the light of his clinical experience. In this therapy, the gentle puncturing and heavy tapping are carried out with the seven-star needle until light bleeding occurs, followed by drawing out of pituita or stagnant blood and coagula by cupping, in an attempt to promote tissue regeneration by removing blood stasis for promoting the flow of qi and blood in the meridians. The therapy has been developed by Dr. Yang Yifang (the author's father) in clinical practice over the past six decades. Satisfactory therapeutic results have been obtained in clinical practice, which are introduced with examples below.

    Both therapies of traditional crude herb moxibustion and drug acupoint application were used in 209 cases of allergic asthma to compare their long-term and short-term therapeutic effects and to analyze the relationship between clinic therapeutic effects of both therapies and differential types of the disease. The results showed that the short-term total effective rate in the group of drug acupoint application was higher than that in the group of traditional crude herb moxibustion, the therapeutic effects of drug acupoint application being closely related to differential types. Analysis also shows the best short-term therapeutic effect was in the type of heat in the lung while the poorest effect in the type of deficiency of the kidney-yang.

    Objective: To evaluate the therapeutic effect of Chinese patent drug Shu Feng Huo Luo Pian (疏风活络片 Pill for Dispelling Wind and Activating Collaterals) for the treatment of osseous arthritis. Method: 50 cases of osseous arthritis were divided randomly into two groups, the Shu Feng Huo Luo Pian group (the experimental group), including 30 cases (aged 63.5±4 years), treated with 2 such pills, po, bid; and the control group, including 19 cases (aged 63±5 years), treated with Sulindac 0.2g, po, bid. The two groups were all supplemented by medication of calcium Caltrate D 0.6g, po, qd. The above-mentioned medications were administered for 2 courses, 2 weeks constituting a course. Result: The total effective rate of the experimental group evaluated by the doctors was 83.3%, and that evaluated by the patients was 90%, with mild side effects. Conclusion: Shu Feng Huo Luo Pian is an effective Chinese patent drug for treating osseous arthritis, with less and mild side effects. Osseous arthritis is characterized mainly by chronic arthralgia, ankylosis, and arthroncus accompanied with dysfunction of the joint. It affects predominantly the movable joints of the human body, such as the joint of the hip, knee, ankle, elbow, and finger. It is a commonly seen disease, which mostly affects the life activities of the aged people. Its morbidity rises along with age. X-ray mass survey has shown that the morbidity of this disease among people aged 15-24 years is about 10%, while that among people aged 55 years is about 80%1. As China is entering an aged society, due attention should be paid to this kind of disease. This article offers a clinical observation on the therapeutic effect of Shu Feng Huo Luo Pian (疏风活络片 Pill for Dispelling Wind and Activating Collaterals) for 50 cases of osseous arthritis as reported below.

    Integration of acupuncture and drug is a therapeutic method in which acupuncture treatment and pharmacotherapy are synchronously performed in the same patient to prevent and treat the disease. Its importance can be traced back to Sun Simiao (581-682 A.D.), a prominent physician of China's Tang Dynasty. He pointed out that: A physician who masters only acupuncture treatment in clinical practice but not moxibustion, or masters only moxibustion treatment but not acupuncture is not a skillful doctor; and a physician who is only aware of the application of acupuncture and moxibustion but ignorant of pharmacotherapy, or is only aware of pharmacotherapy but ignorant of the application of acupuncture and moxibustion also should not be a skillful one. Not only should a skillful doctor have the knack of applying acupuncture and moxibustion, but he should have the knack at pharmacotherapy in treating diseases. Each of acupuncture and moxibustion, and drug has its strong points. Therefore, both of them should be used simultaneously, thus bringing the strong points of both individuals into full play and offsetting their weaknesses so as to obtain a satisfactory therapeutic effect. With the development of modern medicine and introduction of advanced technology, there have been brilliant developments in the method and applied range of integration of acupuncture and drug, particularly in the studies on combined anesthesia induced by using acupuncture and drug, and administering drug by acupoint. All these have provided us a ground to further probe into the mechanism underlying the integration of acupuncture and drug.

    63 patients with senile vascular dementia were randomly divided into the treatment group (treated by acupuncture) and the control group (treated with piracetam). The authors observed the changes in the score of Hasegawa's dementia scale (HDS), p300, rheoencephalogram, topographic EEG, superoxide dismutase (SOD) activity in erythrocytes, and lipid peroxide (LPO) level in plasma before and after treatment. The statistical data showed that the total effective rate in the treatment group (80.6%) was significantly higher than that in the control group (25%), and the differences in the observed indexes before and after treatment were significant (P<0.05 or P<0.01) in the treatment group but not in the control group (P>0.05), indicating that the acupuncture treatment was superior in immediate therapeutic effect on senile vascular dementia to drug treatment.

    The Ji De Sheng She Yao tablet (季德胜蛇药片)is a drug for snake-bites and insect bites. According to recent reports, this drug has been used to treat herpes zoster, hepatitis B, parotitis, nephrotic syndrome, acute conjunctivitis, recurrent aphtha, urticaria, etc.1 with good therapeutic effects. More than half of the AIDS patients suffered from herpes zoster. The area of skin lesion in AIDS patients is larger than that of the patients without AIDS and the pain lasts longer. Many cases have keloids and depigmentation for months, even severe pain till their death. Routine therapeutic methods are not satisfactory in therapeutic effects. The expert group of traditional Chinese medicine for cooperative research on AIDS used Ji De Sheng She Yao tablets (季德胜蛇药片) to treat AIDS complicated by herpes zoster for the first time and the results are reported as follows.
