Abstract:

目的: 我们旨在比较克拉霉素和阿奇霉素治疗鸟分枝杆菌复合肺病 (MAC-LD) 患者的因不良事件和治疗结果导致的停药率。 方法: 在 2001-2013 年诊断为 MAC-LD 的患者中，选择 560 例在 2018年5月前开始治疗作为指导治疗的患者进行不良事件分析。其中，选择 316 例接受治疗 ≥ 12 个月的患者进行结局分析。对他们的医疗记录进行回顾性审查。使用治疗加权的逆概率 (IPTW) 方法进行调整后，分析停药和治疗成功率。 结果: 在 560 例患者中，分别有 466 例 (83.2%) 和 94 例 (16.8%) 开始使用含 CLR 和 AZM 的方案。使用倾向评分的 IPTW 方法显示，CLR 使用归因于不良事件的停药率显著高于 AZM 使用 (24.6% vs.9.6%; P = 0.001)。接受指导治疗 ≥ 12 个月的 316 例患者的总体治疗成功率为 83.2%。经 IPTW 方法校正后的分析显示，使用 CLR 和 AZM 的治疗成功率无显著差异; 此外, 两种药物的 1 年和 3 年复发率相似 (6.8% vs. 6.0%; P> 0.999 和 31.0% vs. 37.5%; P = 0.482，分别)。 结论: 这些研究结果表明，含 AZM 的方案可能是治疗 MAC-LD 的更好的初始选择，因为它导致不良事件导致的停药率较低，同时提供了与 CLR。
