Abstract:

: 引言和目的: 良性输尿管狭窄的重建手术和长期肾造瘘术往往是侵入性的，导致生活质量差。球囊扩张有可能弥合这一差距。我们介绍了我们系列的结果，并检查了狭窄复发的危险因素。材料和方法: 在我们的单中心回顾性队列分析中，2012年8月至 2018年7月我们系列的 109 个狭窄。所有狭窄均向后或前倾扩张，随后行支架置入术。随访影像学评估狭窄复发。结果: 患者平均年龄 57.7 岁 (SD ± 12.6)。平均随访 20.2 个月 (SE ± 1.8)。所有狭窄均成功扩张和支架置入。总体而言，平均随访 63.7% 个月 (SE ± 20.2) 时，平均通畅率为 1.8。与医源性原因 28.0% (21/75) 和放疗 63.6% (7/11) 相比，结石/炎症引起的狭窄有 100.0% (5/5) 的复发风险 (p = 0.001)。非偶发狭窄在 57.4% (27/47) 时的复发风险也显著高于偶发狭窄 13.6% (6/44) (p = 0.000)。复发组狭窄平均长度为 12.5mm (SE ± 1.7)，无复发组为 9.6mm (SE ± 0.7) (p = 0.001)。同侧萎缩肾的存在与 72.2% (13/18) 复发风险与非萎缩肾的 27.4% (20/73) 相关 (p = 0.000)。复发组和未复发组狭窄的平均年龄分别为 14.5 个月 (SE ± 4.6) 和 5.2 个月 (SE ± 2.1) (p = 0.013)。结论: 球囊扩张治疗输尿管良性狭窄是可行的。它的作用可以在选定的患者中持久，即与正常肾脏的非照射，偶然，短狭窄。这将使不适合重建手术的患者受益。
