使用通用数据模型分析电子健康档案中的抗癫痫药物相关不良反应。

Abstract:

目的: 抗癫痫药物 (ASDs) 可引起广泛的药物不良反应 (adr)。最近，使用通用数据模型 (CDM) 的电子医疗数据已经被引入并普遍用于药物警戒研究。我们旨在使用 CDM 分析 ASD 相关 ADR，并评估 CDM 分析在单个三级医院监测 ADR 的可行性。方法: 我们选择了 5 个 asd: 奥卡西平 (OXC) 、拉莫三嗪 (LTG) 、左乙拉西坦 (LEV) 、丙戊酸 (VPA) 和托吡酯 (TPM)。纳入 18 岁前诊断为癫痫并接受单一疗法治疗的 asd 患者。我们测量了四种 ADR 结局 :( 1) 血液学异常，(2) 低钠血症，(3) 肝酶升高，(4) 亚临床甲状腺功能减退。我们进行了亚组分析，以排除合并用药的影响。结果: 从数据库中，1344 例患者被纳入研究。1344 例患者中，436 例接受 OXC，293 例接受 LTG，275 例接受 LEV，180 例接受 VPA，160 例接受 TPM。14.1% 服用 VPA 的患者发生血小板减少症。10.5% 的患者服用 OXC 发生低钠血症。在 19.3% 服用 VPA 的患者中检测到肝酶升高的可变范围。亚临床甲状腺功能减退症发生在大约 21.5% 至 28% 的 ASD 单药治疗患者中，根据 ASD 的类型没有显著差异。在亚组分析中，我们观察到相似的 ADR 趋势，但 TPM 组血小板减少较少。意义: CDM 评价的 adr 发生率和趋势与以往文献相似。CDM 可以成为多中心研究中分析 ASD 相关 adr 的有用工具。应认真解决清洁发展机制的优势和局限性。
