膀胱癌根治性膀胱切除术后术中去甲肾上腺素给药与癌症相关结局: 一项队列研究。

Abstract:

背景: 麻醉技术对癌症复发的影响是有争议的。血浆儿茶酚胺水平升高与多种癌症的血管生成和转移有关。 目的: 评估接受根治性膀胱切除术伴尿流改道的肌层浸润性膀胱癌患者术中持续给予去甲肾上腺素与肿瘤相关结局之间的潜在相关性。 设计: 回顾性观察性队列研究。 单位: 单三级中心，2000年 2017年。 患者: 我们连续纳入了 1120 例接受根治性膀胱切除术和尿流改道的尿路上皮癌患者，其中 411/1120 例 (37%) 患者接受了术中持续给予超过 2 μ g kg bw h 去甲肾上腺素。 主要结局指标: 主要结局为术后 5 年内肿瘤复发时间，死亡为竞争结局。我们使用治疗加权的逆概率来调整治疗组之间的不平衡，一个组接受了超过 2 μ g 的 kg-bw-h 去甲肾上腺素，另一个组接受的较少。我们进一步调整了术中变量或手术年限作为敏感性分析。 结果: 持续服用 2 μ g 以上的去甲肾上腺素使肿瘤复发轻微增加 (风险比: 1.47，95% CI 0.98 ~ 2.21; p = 0.061)。调整术中变量后，手术风险比为 1.82 (95% CI 1.13 ~ 2.91，p = 0.013) 和 1.85 (95% CI 1.12 ~ 3.07, p = 0.017)。总体死亡率 (伴或不伴肿瘤复发) 不受去甲肾上腺素的影响 (风险比: 0.84，95% CI 0.65 ~ 1.08，p = 0.170)。 结论: 如果根据术中变量和手术年份进行调整，持续给予超过 2 μ g kg bw h 的去甲肾上腺素与肿瘤复发风险比略有增加相关。这一观察结果可能反映了术中去甲肾上腺素的低潜在致癌作用。 试用注册: 不适用。
