Abstract:

目的: 本研究的目的是检查先前使用硝基咪唑类抗生素治疗对幽门螺杆菌治疗成功的影响。 背景: 先前的硝基咪唑暴露可能增加硝基咪唑耐药幽门螺杆菌的可能性。目前的幽门螺杆菌治疗指南建议，在没有药敏试验的情况下，先前接触硝基咪唑的患者不应使用硝基咪唑抗生素治疗。缺乏支持这一建议的数据。 研究: 我们检索了 Clalit 健康服务数据库，以确定 2015 和 2010年首次进行 C-尿素呼吸试验的 25 至 60 岁受试者。既往接受 H. pylori 粪便抗原检测或胃镜检查的患者被排除在外。检索药房调配数据。 结果: 共有 1386 例受试者 (34.8% 例男性，年龄 40.7 ± 10.7 岁) 接受了含硝基咪唑的方案，包括 282 例 (20.4%) 既往有硝基咪唑暴露。在既往有硝基咪唑暴露和无硝基咪唑暴露的受试者中，分别有 58.9% 和 73.8% 获得了成功根除 (比值比，0.51; 95% 置信区间，0.39-0.67; P<0.0001)。硝基咪唑暴露对硝基咪唑、质子泵抑制剂、阿莫西林或克拉霉素三联疗法的成功产生不利影响 (39.4% vs. 63.4% 和 54.4% vs. 73.6%，P<0.01，分别)，但不是四联疗法。经过多变量分析，硝基咪唑暴露与根除失败显著相关 (比值比，1.89; 95% 置信区间，1.43-2.50; P<0.0001)。在根除成功的受试者中观察到较长的硝基咪唑暴露时间和较低的累积硝基咪唑剂量 (两者均 P<0.0001)。 结论: 硝基咪唑暴露可能对以硝基咪唑为基础的三联疗法的成功产生不利影响，但不会影响四联疗法。临床医生在对幽门螺杆菌感染进行经验性治疗之前，应进行彻底的患者药物史。
