厄洛替尼和二甲双胍治疗转移性三阴性乳腺癌的 1 期研究。

Abstract:

背景: 表皮生长因子受体 (EGFR) 在转移性三阴性乳腺癌 (mTNBC) 中经常过表达。克服 EGFR 抑制耐药的一种策略是同时抑制下游信号。抗糖尿病药物二甲双胍同时抑制 MAPK 和 PI3K/mTOR 通路信号。我们在一项 mTNBC 患者的 1 期研究中评价了厄洛替尼和二甲双胍的组合。 患者和方法: 既往至少接受过 1 次转移性疾病治疗的 mTNBC 患者符合条件。厄洛替尼剂量固定在每日 150 mg。根据 3 + 3 设计计划二甲双胍剂量递增。在治疗的前 5 周内评估剂量限制性毒性 (DLT)。主要目的是确定二甲双胍与固定剂量厄洛替尼的最大耐受剂量。次要终点为反应率、疾病稳定率和无进展生存期。 结果: 入选 8 例患者。转移性疾病既往治疗的中位数为 2.5 例 (范围 1-6)。在 DLT 评估期内未报告 DLT 事件。大多数不良事件为 1/2 级。3 级腹泻尽管有最大限度的支持治疗，但 1 例患者需要减少二甲双胍的剂量。3 级皮疹导致 1 例患者退出研究。未报告 4 级不良事件。观察到的最佳反应是 2 例患者 (25%) 病情稳定。中位无进展生存期为 60 天 (范围 36-61 天)。 结论: 厄洛替尼和二甲双胍在预处理的 mTNBC 患者人群中耐受性良好，但在该人群中未证明疗效。
