中国老年人对未来护理的感知可用性和抑郁症状: 来自 CHARLS 的证据。

Abstract:

背景: 鉴于人口迅速老龄化、家庭结构不断变化和相当大的国内移民率，中国在为老年人口提供足够照顾方面面临的挑战引起了人们的主要关切。在家庭层面，这些社会变化可能会产生护理不确定性，这可能会对老年人的心理健康产生不利影响。本文应用社会支持和控制理论，考察了中国老年人未来护理的感知可用性与心理健康之间的关系，以及这种关系如何受到经济不足、健康脆弱性、和城市/农村背景。方法: 分析基于中国健康和退休纵向研究的数据，这是一项对中国 45 岁及以上人群进行的多面板全国代表性家庭调查。数据是 2013年和 2011 波的研究，初始样本量约为 17,000，其中约 11,000-14,000 的受访者用于我们的最终回归模型。用流行病学研究中心抑郁量表 (CES-D10) 测量两波抑郁症状评分, 2013年，通过询问受访者问题 “假设在未来，你需要在基本日常活动方面得到帮助，比如吃饭或穿衣,你有亲戚或朋友 (除了你的配偶/伴侣) 愿意并且能够在很长一段时间内帮助你 (是/否) 吗？" 结果: 多元回归分析显示，即使在控制了家庭结构、社会经济地位等与护理不确定性混淆的因素后，未来护理支持的不确定性也与更大的抑郁症状相关, 和抑郁的滞后测量。此外，如果没有预期的护理来源的老年人也经历了功能限制，他们的抑郁症状将面临双重危险。结论: 考虑到中国人口的快速老龄化，慢性病负担的预期增加，以及孝道护理的可能减弱, 这一分析表明，中国的老年人可能越来越多地面临健康和社会状况，不利于他们的精神卫生。应该讨论、制定和实施补救这些问题的政策。
