Abstract:

背景和目的: 不明来源的栓塞性卒中 (ESUS) 患者通常表现为轻微症状。我们旨在比较轻度和重度 ESUS 的基线特征，确定重度 ESUS 的预测因子，并评估重度 ESUS 患者的结局。 方法: 在 AF-ESUS (AF-ESUS) 数据集中，我们使用入院时美国国立卫生研究院卒中量表 (NIHSS) 的中位数评分作为临界值对 ESUS 严重程度进行分层。我们进行了多变量逐步回归分析，以确定严重 ESUS 的独立预测因素，并评估 ESUS 严重程度与随访中卒中复发、死亡和新发房颤 (AF) 的相关性。通过 Kaplan-Meier 乘积限制方法估计结局发生率的 10 年累积概率。 结果: 在 772 例患者中 (NIHSS 中位数: 6 (四分位距: 3-12))，414 例 (53.6%) 患者有严重的 ESUS (即NIHSS ≥ 6)。女性是严重 ESUS 的唯一独立预测因子 (比值比: 1.72 (1.27-2.33))。复发率 (3.3%/年 vs. 3.4%/年，调整后的风险比: 1.09 (0.73-1.62)) 和新的事件 AF (13.5% vs. 17.0%，校正比值比: 0.67 (0.44-1.03)) 在重度和轻度 ESUS 之间相似，但死亡率较高 (5.4%/年 vs. 3.7%/年，调整风险比: 1.51 (1.05-2.16)) 在严重的 ESUS。重度和轻度 ESUS 之间卒中复发的 10 年累积概率相似 (38.1% (29.2-48.6) vs. 36.6% (27.8-47.0)，log-rank 检验: 0.01，p = 0.920)。与轻度 ESUS 相比，重度 ESUS 患者的 10 年累积死亡概率更高 (40.5% (32.5-50.0) vs. 分别为 34.0% (26.0-43.6); log-rank 检验: 4.54，p = 0.033)。 结论: 与男性相比，女性有更严重的 ESUS。重度 ESUS 患者的卒中复发率及新发 AF 发生率相似，但与轻度 ESUS 相比，死亡率更高。
