严重高氨血症危重患者的特点和转归。

Abstract:

目的: 确定重症高氨血症患者的病因和预后。 材料和方法: 成人 (18 岁或以上) 回顾性观察性研究 2007年 2016年入住 MICU，血清氨水平> 180 μ mol/L (正常上限的 3 倍)。 结果: 78 例患者 (45 例男性，32 例女性) 的中位年龄为 52 (四分位距 [IQR] 46-58) 岁。高氨血症最常发生在慢加急性肝衰竭 (ACLF) (49%) 或失代偿性肝硬化 (27%) 由于既往胃旁路术 (9% ), 急性肝衰竭 (6% ), 或丙戊酸 (3%))，因此较少见。入院时血清氨水平中位数为 201 μ mol/L (IQR 126-265，范围 18-736)，峰值为 245 μ mol/L (IQR 205-336,范围 185-842)。住院期间死亡 50 例 (64%)。8 例 (10%) 患者出现脑水肿，仅 1 例存活。8 例脑水肿患者中有 6 例存在与 ACLF 相关的高氨血症，在该亚组患者中的发生率为 14%。死亡率和脑水肿均与峰值氨水平无关。 结论: 严重高氨血症的危重患者死亡率高，有发生脑水肿的危险。
