Abstract:

: 由于病毒性下呼吸道感染 (LRTI) 需要住院治疗的婴儿在早期发生复发性呼吸系统疾病和儿童期以后发生哮喘的风险很高。值得注意的是，所有经验证的病毒 LRTI 临床量表都侧重于预测急性严重程度而不是复发。我们提出了一种新的临床方法，将个体危险因素与床边临床参数相结合，预测幼儿病毒性 LRTI 住院后的复发。一项针对幼儿 (≤ 3 岁) 的回顾性纵向队列，旨在定义因 PCR 确诊的病毒 LRTI 而住院后 12 个月内反复呼吸系统疾病的临床预测因素。数据收集是通过电子病历。我们纳入了 138 例病毒性 LRTI 住院患儿。使用自动逐步 logistic 模型选择，我们发现首次住院婴儿复发的最强预测因子是严重早产 (≤ 32 周胎龄,OR = 5.19; 95% ci 1.76 ~ 15.32; p = 0.002) 和加权低氧血症、肋下退缩和喘息的临床评分 (OR = 3.33; 95% ci 1.59 ~ 6.98; p
